Chapter 13: What Is Comprehensive Orthodontic Treatment?

Comprehensive orthodontic treatment — often called "full" treatment or "Phase Two" when it follows an interceptive phase — is what most people picture when they hear the word "orthodontics." It's the stage where all, or nearly all, permanent teeth are present, and treatment focuses on fully aligning the teeth and finalizing the bite. This is typically appropriate from around age eleven onward, and it applies just as much to teenagers as it does to adults — there is no upper age limit for comprehensive orthodontic treatment as long as the gums and supporting bone are healthy.

The Comprehensive Treatment Process, Start to Finish

• Initial consultation and diagnostic records — photographs, X-rays, and often a 3D digital scan — used to plan the exact tooth movements needed.
• Placement of braces, or delivery of the first set of clear aligners.
• Regular checkins, typically every four to eight weeks for braces, or as advised for aligners, to adjust wires or issue new aligner sets.
• Active treatment, typically lasting between twelve and thirty months, depending on the complexity of the case.
• A retention phase afterward, discussed fully in Chapter 18, using retainers to hold the teeth in their new position while the surrounding bone stabilizes.
