Chapter 9A: A Closer Look at Crowding Severity

Not all crowding is treated the same way, and understanding roughly how orthodontists categorize severity can help you follow the reasoning behind a given recommendation.

Mild Crowding

Often described as slight overlap or rotation affecting one or two teeth, with a small overall space deficit. This category is frequently monitored through the remainder of mixed dentition rather than actively treated early, since it often resolves partially as the jaw continues to grow and more permanent teeth erupt.

Moderate Crowding

A noticeably larger space deficit affecting several teeth, sometimes accompanied by visible rotations or teeth erupting outside the normal arch line. This category is more likely to be discussed as a candidate for either interceptive space management or, more commonly, straightforward comprehensive treatment once permanent teeth are in.

Severe Crowding

A significant space deficit, sometimes requiring consideration of selective tooth removal (of specific baby or, in some cases, permanent teeth) as part of the treatment plan to create adequate room for proper alignment. This category most often involves a more detailed, individualized discussion with your orthodontist about the specific plan and reasoning.

Severity categories are a useful shorthand, not a rigid formula — your orthodontist's assessment of your child's specific case, not the category label alone, should guide the actual treatment decision.
